not neccesarily, a lot of animals in the game are not strictly prehistoric, and humans have had diseases for as long as humans have been on the planet. sure, we can probably rule out syphilus until the devs add human breeding to the game, but HIV? could get it from bad transfusions from mistreated prisoners.
Go around, poop in people's bases. They use it for fertilizer, infect their plants, everyone dies.
