Edit - in water or inside multiple boulders or something may cause a wonky view too...
1 you teleported into the ocean, the water throughought the Island/The Center/ Ragnarok is map-wide with the island rising out of it. Underwater there, there's no fog but it still gives you the distorted double vision effect.
2. You teleported near lava on scorched earth while under ground.
3. You teleported into radiation on Aberration.
4. You teleported into the kill barrier on extinction.
