Report a translation problem
Fence foundations have this problems, as do ramps that stop at the door. You can get around the issue by stacking two doors on top of each other. You don't have to open the top door though, you can get thru just by opening the bottom door.
If it is on regular foundations without fence foundations or ramps though I don't know what your problem might be.
It also affects sloped roofs above the doorway.
Doorways can be a bit tricky if you don't build them just right or add something above that clips into the hit box
Rubber banding is sometimes an easy fix. Die and respawn.
Might even work if you fast travel to another bed. Not tried it.
Drove me nuts first time this happened to me.

This is why Batman always drops in via skylight, and never uses the door if he can bust in through a window.
You obviously need more grapling hooks.
