try setting full screen resolution to 1280x720.
i have a 1440p monitor and if set fullscreen to 2560x1440 i get error noise and only see a kega fusion window with a black background, if set it to 1920x1080 the image is cropped and can't see the full image, but if set to 1280x720 it works without issues.
idk about the controller issue. post a screenshot of your fusion configuration/controller window maybe it might help find the problem.
1280x720 didn't work. I have a Ryzen 3800X CPU and a Radeon RX5700 GPU.

One thing that annoys me about joytokey is that it opens the joytokey website whenever you launch the program. But hey, I have it on hand, so there you go.
Not to be confused with joy2key, which is a program for linux with a similar purpose, but lacking a GUI (you'd use qjoypad which is a Qt GUI wrapper for joy2key). There's also antimicro, which is apparently cross platform? I've yet to verify this.

--Find 'ForceFullScreen' under Graphics Compatibility
---Change 32=0 to 32=1
----Save Settings, close fusion settings
-----Reopen Fusion select Full Screen Resolution under Video settings
------Set resolution at '1280x720' (I'm also running with Vsync enabled, scanlines 0%, NTSC aspect enabled, Normal mode, running at 60 FPS in full screen, windows 10, RTX1650, R53600, 5 year old build)
