It has a completely different level line up. And it has mechanics that are unique to CD (although I would like to see the time line levels return, as flawed as they were). And even the special stages were different.
I would say that while I don't think CD is as good as Mania, I do think it feels more like a main entry then Mania does.
As Mania does have some things to call it's own, it does *sometimes* feel like a greatest hits compilation, with some new levels slapped in there (and I am fully aware that the levels are not exactly the same, but they do feel like remixes). It is a good game, and it's a step in the right direction.
But (in my opinion) it is not a sequel. But it is a good, if not a great game even. And I would like to see what else they could do, maybe with more original levels (again my opinion).
But after properly playing through Sonic 4 Episode 2, I think Episode 2 redeems it's self as being worthy of being called Sonic 4. It has less bugs, the quality overall is much better, and it's a more refined experience.
Though I also played Sonic Advance 1-3, which were perfectly good Sonic games and capable of taking the spot as Sonic 4.
Plus as someone else mentioned, you have Sonic CD, which makes Sonic 3 or Sonic CD feel like "Sonic 4". (I don't care which one really, I would say Sonic CD as only to not be confused about Sonic 3 actually being Sonic 4.)
I think the most important thing to say here is that Mania definitely deserves to be apart of the series and fits in with the more classic titles. Almost like a Sonic CD-2.
I also agree that Mania's one drawback is that it's entire theme is built on paying homage to the original games. While that is perfect for Mania and when it released, we need Sega to start working on new content for Classic style Sonic games that don't literally rehash the same level concepts.
Like how many times do we need to see the same zones in each game? If not entirely the same zones by name and design, we get fill-ins with the same exact themes just with different names.
As much as I love seeing Green Hill Zone evolve, especially graphically, I feel like that stage is getting tiresome to see in almost every Sonic game as the first level or an early level.
I mean Mario still sneaks in World 1-1 from SMB in some of their new games, but it's usually an easter egg or secret world content.
Even 5, somewhat felt like a Resident Evil title. And even then it's a really big "somewhat" (Despite that though, I do like the game).
Anyway. My experience with Sonic 4 is fairly limited and I never played episode 2, so I'm not going to judge the quality of the game. As I don't care about the general opinions surrounding Sonic 4, (as the fanbase can be rather fickle regarding what is a good entry). I'll play it someday and make my own mind up about it.
But anyway, a title and/or a number, is not a very good reason at to why a game is a proper follow up, as history has proven many times in the past.
The question wasn't "which is a proper follow up", it was "which is the true Sonic 4".
The proper follow up is neither, by the way. It's Sonic Adventure.

In Sonic CD Metal Sonic was on Eggman's side. In Heroes he was against him... and instead of him. Why? Maybe because of his fall in Sonic CD in the first place? And for Eggman's trying to conquer the world (in the second)?
