At Lost Bastille, if you go through the path, that uses the Antiquated Key in order to skip the Ruin Sentinels (the boarded off wall at the yard near McDuff's workshop). You'll encounter an entirely dark room with a Shadow (humanoid enemy with claws and a beak-like mask) on the wall and a Pharros Contraption. Using a Pharros Lockstone here reveals an illusory wall with a wooden chest behind it, that contains a Soul Vessel.
The good part, if you've already picked it up: Bonfire Ascetics also reset wooden chests. Using one on the Holding Cells bonfire (when you enter from No Man's Wharf) at Lost Bastille resets those.
In any case, messing up a build to the point of not being able to progress (or a softlock even due to bad stats) is simply not possible. Worst case, you farm yourself out of a bad spot.
Not to mention that weapon level ugprades are far more meaningful, than character levels are. Up to Drangleic Castle anyways. HP and ADP is also important (you can mitigate low HP somewhat with infinite Lifegems sold by Melentia), but weapon level will be the biggest help up to that point.
