And you need to beat the game 3 times on a single character to unlock all achievements or waste endless hours with online.
Try going back to all the Creighton and Pate encounters and see if you can find one of them still there. I know I've sometimes missed Creighton in the Shaded Ruins because I hadn't encountered him in Huntsman's Copse first. On my last play through I realized this when I got to Brightstone. I went back to the Shaded Ruins, talked to him, returned to Brightstone, and there he was.
If you just watch, you get nothing. I did that once and got pissed lol.

Later on (about 2 hours in game time or something) a tombstone of Pate will show up in the place where you killed him. Pray (give souls) in front of it and you can talk with his ghost. Nothing useful though.
