Haven't used Flawless Widescreen in particular, but other tools, like Borderless Gaming or iGP11, which hook into the graphic's API in a similar fashion. The game's anti-cheat is not sophisticated enough to detect these. All it cares for are impossible stats and changes to what the game calls "params", which are everything from item stats to merchant inventories. I have changed texture files in the past as well with no issues.
Some more involved mods are also undetectable online, like Blue Acolyte, which provides anti-cheat which actually works and is highly recommended if you want to play online and don't want your save permanently messed up by cheaters, who can put impossible items into your inventory to get your softbanned or cause crashes.
