There are people on here that have said they were banned for using graphics mods, so I would assume using a 3rd party program for UW would flag a ban as well.

You can use Flawless Widescreen[www.flawlesswidescreen.org] and no, you will not get softbanned for using it. In fact, you can safely use all kinds of graphic's API injector type tools, like ReShade, iGP11 (for texture mods, made for DS3, but works with DS2) or tools, that force the game to run in a borderless fullscreen window.

This only applies, if you download a drag & drop mod of unknown origin or if you can't be sure, if it modifies more than it claims to.
The softban system of soulsgames only cares about impossible stats on characters (SM/SL don't match, key items, before it would be possible) and wether or not "params" have been changed, which range from weapon stats to merchant inventories, damage formulas or all kinds of gameplay-related properties and values. DS2 is blissfully unaware of purely cosmetic modifications or graphic's API calls made by other programs.

Like many people, i used Borderless Gaming, ReShade and even directly modded textures via UXM in the past without issues. I'm also using RivaTuner for it's OSD for 10+ years.

Have you tried windowed mode? I've read something about letting Flawless Widescreen do the fullscreen. If it doesn't have that feature, you can try force a borderless fullscreen window, by going into windowed-mode and manually enter the X and Y position of the game's window inside a file, called "userconfig.properties", that's located next to "DarkSoulsII.exe". It's going to require some trial&error, though.

A more "convenient" way, is to use a tool, like "Borderless Gaming" (>free version<[github.com], latest version can be bought on Steam for a few bucks) or "Special K"[www.pcgamingwiki.com]. I put convenient in quotations, because now you'd need two tools.

Come to think of it, if the game doesn't stretch it's image in windowed mode for UW resolutions, you may be able to just get UW working, by forcing a borderless window, without Flawless Widescreen.

Any programs that could interfere with it? I used it ages ago and had no problems with it. Ignore the fact it's 96:9 instead of regular triple monitor, I was messing around and for some reason never uploaded a normal screenshot.

The only thing I could think of is that.. maybe the updated exe is causing issues? Really depends on how much was changed and where. If it's recent comments complaining about the issue, it may be the exe. If it's old comments, likely some random program or messed up config that hinders it from working. Make sure nothing is interfering with the game and try different window modes, like try both fullscreen and windowed/borderless. User Borderless Gaming for a borderless windowed mode. Also, you likely.. maybe.. still have to use nvidia surround to get it working at all. As in, have 1 big monitor virtually. If you don't enable surround it may just not work.

Edit: Also I am stupid, OP has a 21:9 monitor, not multi-monitor. Either way.

That works for some games, yeah. It works for Little Witch Nobeta for example.

Okay thanks, got it working with borderless gaming, UI is a bit stretched but it will have to do.

I have a few even wider ones. No clue why I didn't save a normal 48:9 screenshot though. I only have these wacky ones for some reason.
