Just go with the flow, visit each area when you want to, if it feels like you're still under leveled then just go back
About the order it's also the same thing, it really depends on what you want out of the game. I could say you should start on place A then go B then C but maybe that's the best way for me and not for you
Well, my best advice would be for you to play at your own pace as there are many many ways to complete the game, and most of all, have fun
Around SL 120 should be enough to beat the DLCs. I'd say that Brume Tower is the least path resistant to players, but as the guy above said this could apply only for my personal experience.
