Sanctum Mace build?

Haven't played it in forever, what stats do you suggest I use? And what class should I start out with?

50 Vigor, 40 Strength, Adp till you have 99 Agility. Pump the rest into Endurance, you should be able to get well over 50. Or split it a bit so that you can still level Vit in order to have a lower equipmentload.
Start as Knight, you don't need Dex, Atn, Int and Fth.
Crest of the Rat, Life Ring +3, Poisonbite Ring +1, Ring of Resistance +1.
Shadow Mask, Monastery Longskirt, Sanctum Soldier Gauntlets, Monastery Skirt (highest Poison Def armor + Poison buildup bonus from the gloves).
This gives you over 900 Poison Resistance, making you literally immune. Of course that's just shenanigans and overkill, if you aren't terribad at managing attacks a lot less is also fine. You could equip different rings, i.e. Ring of Blades +2 and/or Flynn's Ring or you could equip heavier armor with Poise (and preferably still decent Poison Def).
Oh, and buff the mace with a Rotten Pine Resin.

It's bad as a poison tool, do not infuse it. Use rotten pine resine + common fruit + poisonbite ring+1 on the common strength build, it's far stronger cause 2h runnings and backstab stacks poison pretty fast and attempt to use moss is punishable cause of massive physical damage

What's a good offhand weapon? And outside of the mace, what are some good poison build ups? How viable is it for PvE?

Poison is situational in PvE, a lot of enemies has high resistances or even immune to poison. I recommend u to not mess with offhand, it's really optional. Some small shield or caestus

Mytha's Bentblades are ♥♥♥♥.

I'd just go with a Poison Heide Knight Sword or any shield.

It's an okay build for PvE. The SM has some good physical damage so you aren't screwed against poison-resistant foes. It gets a bit harder to manage the self-inflicted poison but like I suggested you can simply make yourself immune to poison or close to immune.

There is no better setup for poison in PvP than offhand poison heide lance or pate's spear + poison heide sword or broken thief sword
