When launching multiplayer, you are presented with the lobby where your portrait is displayed to the left. To its right are three large panels with a plus symbol. Hovering over one gives the option of adding a player or AI. This would intuitively seem the correct thing to do, but it isn't. Beneath your portrait is your name in a wide short panel. Beneath this are three more small panels, also with plus symbols. These are the ones you need to add additional players to, not the large panels above.
Hope this clears it up for someone.
[EDIT] Got the panels backward. Do the opposite, use the large panels.
For anyone who may be checking out this thread, I just wanted to make it clear that Anno 1404 (the original non-history edition version of the game) is no longer playable online via our official servers.
There may be some workarounds posted about this using server emulator programs players have posted in these forums, but this isn't something we are able to advise on being a 3rd party program.
Currently, only the Anno 1404 History Edition version of the game is able to connect to our official online servers, with the rest of the History Edition games in the series too.

So those seeking help, do exactly opposite of what I wrote :)
