If you have outposts, sometimes other members of your enclave go out scavenging and can be killed or infected badly and die from it. Time does not stop when you are not in the game, it just slows down.
Some members also will spend time wandering away or leaving outright because they are unhappy. Happiness is helped along by good medical care, a secure compound, successful missions, high leadership, and enough food, weapons, and ammo...among other things.
One way to help slow the carnage is to switch characters more often. When you examine the members of your enclave, it tells you who is home and who is away. This will often allow you to recall those who are off on a mission. There are also mods that keep your most-skilled survivors from going on missions.
