Story Mode: it's the game with a basic story and a bunch of missions to it. It kind of leads you through the game and teaches you how to play.
Breakdown: is the same area (Trumbull Valley) in sandbox mode. You start at Breakdown level 1 and your only real mission is to repair an RV and get out of there. You can do whatever you want in the mean time though - collect survivors, build your base, scavenge the valley dry, &c. When you do pile into the RV and move on, you simply start again at the next level up, and things are harder. You keep doing that until you're ready to lose your mind.
Lifeline: is a whole other game. Same basic mechanics, but you're playing with a story again, and playing as some military folk with a base out along the outer belt of the city of Danforth. So new things to explore, a few new issues, and other "quests" to complete.
