As for "out of favour", a general gets that if he enters a city with negative public order. On the flip side if your general enters a city with positive public order, he'll randomly get the "popular" trait
If you want your faction leader to get the "kind" and all those nice dandy traits, you have to ransom prisoners
"Blind" is almost a guarantee if your general becomes too old
Most of these are randomly gained tho
If you kick your generals or high king you can pick up some nice random traits, along with some possible negative ones.
