Since there's no DRM in the GOG version of KOTOR, couldn't you just install the GOG version of KOTOR and use that executable instead of using files that might get you in trouble with Steam? Or is that also against the Steam rules?
Also, this UniWS ... it works for 16:9 resolutions but probably not for ultrawide resolutions like 3440x1440 right?
The GOG exe may work if it's the v1.03 executable.
For WQhd resolution, you must edit the settings manually and experiment, I didn't tried myself because I only played the game on a 1080p screen.

regarding the graphic options...
anyone know why this line is in there;
DisableVisualBuffers=1
? Cuz, my ini file did not have that line in it and not even sure it 'needs' to have that line in it.
Also, the OP's graphic options have it set to disable soft shadows but, then they have another line (also not in my ini file) called 'AllowSoftShadows=1' which 'seems' like it maybe contradictory to the line 'Soft Shadows=0' earlier in the list... no? According to some guides... that 'Soft Shadows' option 'should' be disabled in order for the game to run a bit smoother / successfully on modern systems (or set to 0) but, I can't confirm that... don't seem to remember changing that to disabled in my past experiences of playing the game. It maybe just a trial and error thing for those with issues.
