When I landed on Manaan, as soon as I got off my ship, Xor was there to attack Juhani. I killed him, then went and got Carth after. We went up and down the dock and all around the Ebon Hawk, but nothing. Tried the same on the other planets, except Kash, since that one is now post Wookie Rebellion. I know the Xor thing is a bug, so I am hoping I'm not locked out for good.
Try going back to your ship on Korriban or Tatooine, leaving with him in party, see if it triggers. I don't think you can usually trigger more than one such quest opening without re-entering your ship and re-exiting again with a different party member. Also if you had him in party when Juhani and Canderous and whoever else had meet-and-greet quest advancing, then maybe you didn't trigger Carth's because you triggered other companion's.
You must trigger the quest before your encounter with the Leviathan. I went from Dantooine to Kassyk, then Tatooine, then Manaan. On Manaan I got Carth to speak about his family, but on the way back to the Hawk, I switched him out with Candorus to trigger his quest. Right after that the Leviathan caught us and after that Carth's quest didn't trigger. When I loaded back an earlier save, it did..
Also, you can trigger two companion quests at the same time. On this instance I triggered Juhani's and Candorus' quest without switching anyone out, getting back to the ship or moving back and forth.
