Missing Companion Quest

I have this really weird glitch in KOTOR. When Mission Vao joins, her companion quest fails to appear in my list of Active quests.

I have had this glitch since I had problems with KOTOR. I have installed and reinstalled the game several times for other problems. I deleted my save files and began a new game multiple times. Also whenever I verify the interity of the game files, one file fails to verify.
