No exit portal

Is there any rule for exit portals to appear, e.g. time and place?

Never encountered this. But you can buy a Portal Flame at the Vendor. For cases like this i guess.

Last three runs i had only one gold portal on map, and no green one. And the gold portal was always at the other side of the map, and the enemy count has seemed to triple since I went from level 6 to level 7.

Green portals are ONLY GUARANTEED until you use them, then they become potential spawn points instead and the ONLY way out is the gold(itinerant) portal or, as previously stated, killing a boss. That or stopping a Calamity from starting lets you create a temporary portal home.

Yup, had the same thing happen to me several times. Started playing yesterday.

It has to be a rare bug. It had happened several times to me on the first day of playing too. After that, I am now lvl 22, it never occurred again.
