I guess you eat too much coconut? If you eat more than 3-4 coconut a day you got poisoning.

I think i know the cause of this, early in the game it said"not feeling too good" after stepping on a flat purple spiked organism on the beach, when pulling up the raft on to the beach. Read in the comments that the pipi plant together with a specially crafted coconut can act as a antidote. Is this true? Ty.

Food Poisoning and actual Poisoning are two different things. They have different causes and cures:

“Food Poisoning” is a penalty for eating bad food like raw meat or anything that’s spoiled. It induces periodic vomiting which damages your vitals. It’ll go away on its own.

“Poisoning” is accompanied by a message saying ‘I don’t feel so good’. It’s caused by bumping into any number of venomous creatures which are almost 100% water-bound (Crown-of-Thorns Starfish, Sea Urchins, Lionfish, and Sea Kraits) or randomly when getting bitten by a snake. It damages your health over time and needs to be cured by an Antidote.

Ty, for replying. So in this case, the pipi plant together with a specially crafted coconut can act as a antidote. Is this true? Do you know? Ty.

Yes. Combine a coconut flask with a pipi plant to make an antidote. You can usually find two pipi plants on each island, and you should save at least one to replant at your home base. You're going to want to start a garden as soon as you can because plant resources are finite (if I'm remembering correctly).

Snakes, sea snakes, lionfish and those purple starfish will all poison you.
Protip: once you're poisoned, you cannot be poisoned again, so that's a good time to go hunting for lionfish, which you use to make shark repellant.

The heart icon on the watch is your health if that reaches zero you die, you can regen health if your water and food stats are high and as long as your not bleeding or poisoned, at the start of the game it’s possible to get poisoned by the purple star fish and you won’t have unlocked the ability to make the antidote yet so be careful, usually on the first island there is a pipi plant collect it and make a farming plot and put it in there so you can start to farm the pipi plants every couple days as the ones on the ground don’t grow back, once you have the ability to make the antidote then your able to cure your poisoning so it’s less stressful from that point

I think poisoning also goes away on its own after a while. You just have to make sure you have enough health to outlast it.
To add to the full list (that I'm aware of) for poisoning...

Every Isle (on land):
Near shore and offshore: purple spiky things (avoid)
Near shore and offshore: green glowy things (avoid)
Geek tip: if it's your 'home island', what I try to do is build a foundation exactly over these poisonous things... where that foundation is up off the ground a few inches... but not so high that the character can walk under the foundation. That way... even if I'm running around in the dark, I can't possibly run over it.

Every isle (in the sea):
Sea Snakes (cannot kill, just avoid)
Lionfish (either avoid, or hunt with fishing spear)
grey-ish spiky coral; it's actually kind of rare... but if you swim into it... (avoid)

Isles with large cliffs (on land):
venomous snakes (kill with 1 refined spear, or 2 crude spears)

As already mentioned in this thread, gather pipi as soon as you can and/or farm it as soon as you can. No matter how good you might be at avoiding the poisonous things... when fleeing some other danger (i.e., giant hogs or giant crabs or sharks or... or.. or..) is really easy to run right through or toward something poisonous

It's been a long while since I've played (about a year or so), but I don't remember either of those things.
The greyish spiky coral is pretty rare. so rare that it was a long time before i first got poisoned by it. I actually wondered "what poisoned me, there's nothing here"??? oh, what's this. huh.

It's possible that since it's in medium-ish off shore (about the range or just beyond where clay nodes are found), if you were poisoned by it, you could have easily thought "Sea Snake!"
