"Cave Island" and "Cave Island II: Labyrinth." I suggest you read their descriptions in the workshop to make sure they're what you're looking for. Although they sound similar, they are very different islands. For example, Cave Island II doesn't have any animals on it, but there are some very challenging deep water, cave wrecks to dive—which you will quickly die from if you don't go in with right equipment or manage your time properly.
You can find them in the Steam Workshop. The easiest place to find them is in the "Most Recent" category on page 1 at the bottom of the page, and page 2 at the top of the page.
I also played on a very challenging and fun island recently built by RobertMonkey. It's called, "The Rock." I described what happened to me on the island in this section of the forum (General Discussions). Look for the title, "Looking for more adventures like this one..." It's on page 2 near the top of the page. To some, my experience may have seemed like a grueling session in the game to avoid. To me, it's what survival games should be like. Give it a try, I think you'll like it.
Also, if you read further down in that post… other players have suggested different islands to try that are challenging and built by different people—such as deezmaps by deeznutz, and the website nexusmods.com.
There you go… There are some options for you if you want this game to be more challenging. Good luck!
I've been to some custom islands where loot is just thrown all over the beach—like a junkyard. It does ruin the game, which is why you won't find that on my islands. I try to balance the loot as much as possible, and you have to search for it. Otherwise, what's the point?
Give Cave Island a try. I think you'll like it... Same goes for The Rock. If you don't like the islands, just leave. There's nothing this says you have to finish an island once you land on the beach. On the other hand, if you never give any custom islands a chance to meet your needs… you're never going to enjoy the game. So if you try it and don't like it, you're in the same position that you are right now. You've lost nothing but a small amount of time.
Just how I see it…
I only do it in extreme cases, on islands I truly love for other reasons and want to include in my world, and I hope that most creators won't mind since I do it because I appreciate their creation.
