But yellow means they are alerted, or suspicious. They go from white dots to yellow and you hear them shouting about taking cover, etc, etc. If you have them tagged you can see them ducking for cover or running. After a while you get told 'Enemy Searching' then finally you get the all clear and they go back to white dots.

It happens if you have multiple generators running at the same time. (Probably because they share the same sound file).
If you have more than three running concurrently and visit the previous ones, you'll notice that they begin to fall out of sync - more so, with each new generator added, until they suffer a catastrophic failure at around 8 or 9 generators.
Saving and reloading the game has absolutely no effect on this behaviour whatsoever - to the extent that it may be thoroughly tested, whether utilising saves, or not. (In other words, while broken. The behaviour is very robust and well behaved - it can't be altered, tricked or changed by any combination or reloading, restarting, rebooting etc. (the save will simply pick up where you left off).
The way to overcome it, is to destroy each generator, once it's served its purpose.
If you plan to only use say, 3 or 4 generators over the course of a map. Destroying each one, while ideal, is not strictly necessary.
If you seek ghost kills take them out where they are isolated or lure them into a blind spot with your trusty stone.
