How the hell do you complete the game on that difficulty? I am trying to get the achievements and it is so difficult

It is difficult, no question. But very doable. You'll want to be very familiar with the maps beforehand and give yourself plenty of time. Ask for mission-specific advice here, too; lots of folks willing to help.

One tactic that helped me on some maps like the Dockyard and Allagra was to use the AI's overaggressiveness against it. Tease them out and get them to attack you where they are exposed. The bridge on San Cellini island might be one such spot. Another tactic is to kill them from range. If you snipe them from more than 100m away they seem to have a lot of trouble zeroing in on your location. Others might disagree with that, but it has seemed that way to me. Another option is to go coop. I think you still get all the achievements. Good luck!

As someone who has completed the campaign and all DLC's on Authentic + (with no saves),
I would suggest that you just focus on the Main Ojectives to complete the level.
Use long range sniper (sound masked and suppressed) and stealth, avoid "Rambo" style (Run &Gun) as much as possible.
Shoot Officers, Snipers and Spotters first.

It's doable, I just finished the game on Authentic Plus (no saves).

-Take your time and find kill zones, where you can booby-trap the way to you and just snipe enemies as they come for you.
-You have all the time in the world, wait and listen.
-Highground is your friend.
-Only go for Main Objectives.
-Don't be afraid of using a secondary weapon, use the noise and pull enemies in = learn when to Rambo.
-Know the map and make a plan.
-Really learn how to sniper, 100-150m is the sweet spot to kill enemies at, above 150m only Snipers are a problem for you, anything else will move closer to you at some point.
-Skipping or stealthing as much as you can, will help out on a lot of the maps.
-A fast rute means if you die, you can get back to the point quite fast and try again.
-See it as a challenge, you will die.. try to learn from it.

San Cellini island is imo a really soul crushing map to start on, most likely it's you first lvl to play on Authentic Plus, it's really open, have a lot of killing objectives and lacks good places to funnel enemies towards you.
On top of that you're learning to snipe with both wind and bullet drop, if you made the jump from Cadet to Authentic Plus.

So maybe try another map first?

I was lucky and got San Cellini island on 2nd try, but it took me about 1h20m and I killed almost everything.. compared to finishing Allagra Fortress under 30 minutes.

Just keep at it m8, you will get better with every attempt. Good luck with it :D

Last week I've finished the campaign in Authentic + with no saves and I can assure you it's an absolute satisfying experience. Why do I tell you this? Just because the first rule to achieve success in here is to try hard, no matter what, and, though sometimes it's not easy, never get frustrated. You just gotta try to have fun: the experience is totally worth it.
I agree with each and every advice they gave you here although I wouldn't try to avoid secondary objectives. I found that the easiest and less anguishing way to deal with certain maps is to kill everyone and often completing certain secondary objectives helps a lot. I would also recommend to start by another map other than San Cellini: it's not the hardest but certainly it's a time and patience demanding one. What worked for me in that one is sniping at a safe distance to kill every primary target. It's a 200 meters and something shot but you can kill the first one from the first vantage point. Find a safe and distant spot to kill each one and in doing so you'll not only kill him but also everyone around him.
As far as I know there are two way to approach Schmidt: sneaking to his house without being detected or breaking havoc from outside and taking him down when he tries to escape. I found the second option the easiest one for me. Let us know if you succed and, above all, have fun!

Yesterday I finished "Better than the best" achievement. My tactics was about the same on most maps: there are specific zones on each map where you can shoot all without alerting the other areas - find a good spot and wait for the enemy until all are dead. You can lure more in by looting bodies. Then advance to the next area, get to a good camp position with silencers and then make noise.

Main thing is to focus on main tasks.

For example in Mansion map, go to left from the start point towards the bridge on the north, kill all in that park area http://sniperelite4maps.de/g/#4/-77.44/-1.19 (about 15 kills, wait for all), then go to the brídge with a silencer (easy boat kill there), and kill all around the church with silencers. After that go to the opening of the fence on the other side of the church and camp there http://sniperelite4maps.de/g/#5/-80.820/-55.042 without silencer and kill all in the mansion (they will come to you), including Rothbauer (which will get you "death from afar" achievement. Then go to mansion and for the the other side of the mansion there is a second floor balcony/staircase right under the deadeye statue http://sniperelite4maps.de/g/#5/-75.547/-64.973 where you can snipe the sniper (target glint) from 375m (about 5-10 attempts needed usually, but he can not get you from that far). You need to watch for about 7 germans coming to you from the driveway (you can tripmine that) while you shoot. Then go to start of the bridge on south-west http://sniperelite4maps.de/g/#4/-71.29/-97.69, snipe all around that house on the other side of the bridge and go on from there straight to around the left side of the battle area in the woods to the bunker where you can "find what Rothbauer is hiding" http://sniperelite4maps.de/g/#3/-43.39/-121.73/m=-22.269,-130.693 . After that sneak to the end point, without shooting anyone. The end point is just in front of the sniper nest (who you already killed). http://sniperelite4maps.de/g/#4/-8.02/-85.83/m=6.053,-88.682

I did it, but boy it was a tough learning curve initially. I never thought i'd do it, it was brutal. But after much playing and getting familiar with routes and primary objective locations it starts to become pretty straight forward.

One tip though, if you wanna stealth kill do not use a weapon at all, even suppressed ammo alerts enemies around the area. Melee stealth does not alert anyone, assuming they are out of the line of sight of others. Just Melee stealth, and hide body if need be.

Another tip, Co-operative counts towards the achievement. As long as the host doesn't manual save at all. So be the host to make sure of that. It does make it all far easier, assuming you haven't got an utter noob helping you. Otherwise it can be harder than being solo, as you constantly worrying about your companion.

Tip 3: For most missions, do what i do and simply find a nice safe spot (usually at spawn point), and make as much noise as possible by blowing up trucks or barrels. The majority of the enemy will come towards you, so you can just pick them off with your sniper easily. Be extra patient though, some do take a while to run to your location. Once your sure you killed everyone, move to next area and repeat. This is by far the easiest solution for Authentic Plus (no manual save). Since you can then move about and do the objectives with less worry, but still be alert as you do have a habit of the odd enemy springing out of nowhere (as they were hiding behind a distant wall).
