I've beaten all difficulties plus both endless, still don't have it filled? What am I missing?

Weapons probably, or effect like Stun or Vortex or Aura which you get when again buying or combining weapon. You can always join in Discord. Developer or other guys can help you with it. There also wiki, in internet, which they said is filled, might see what you are missing.

Did you kill Azrael? He is the big stationary boss in the pit in act 4. He was my last enemy codex entry.

Hmm, might be a bug, this achievement was completed without any problems before. How many hours in the game do you have?

I have this same problem, with over 50hrs played. One of a handful achievements i do not have.

I think as TheComplexByte suggested you might be missing Azrael. This is a boss that is almost impossible to kill when you don't have passive upgrades and very good build. The easiest way would be going back to Mercy difficulty (act4-level12)

So yeah turns out I was just missing Azrael. I swear I killed him multiple times before but whatever lol.
