windows comes with default security settings. neither steam nor the game installer nor the game make any changes nor bypass whatever antivirus, security settings, folder permissions, running programs you have on your system. up to YOU to manage all that for this game and any other game/app you install.
game launcher needs to create folder
Documents\ElderScrolls Online\live
folder should contain UserSettings.txt and folders AddOns Errors Logs SavedVariables Screenshots
if these files/folders are missing
check antivirus log? might be blocking game/game launcher from writing to disk
check firewall log? might be blocking the launcher or game from accessing internet
check windows security/controlled folder access.
if on add exceptions for game launcher/game.
running Windows One Drive? one drive with default settings moves files out of your Documents\Elder Scroll Online folder. so if your launcher creates settings files, the game itself has nothing to read. either disable/uninstall one drive. or if you need it/use it, windows one drive was released in 2007. VERY many websites with tutorials/manuals/movies/youtube videos written by experts devoted to One Drive/synching files/using steam, etc. try leaving a local copy of the ESO settings files.
check windows event viewer and also windows reliability history.
might be a game crash event or system related problems.

While I haven't experienced most of the problems I've read about others having, the biggest one that came up from reading is that ESO has a habit of becoming corrupted on install. The repair function is usually pretty reliable on fixing this and reinstalling is generally not necessary.
My major issue with ESO on Steam was that I would run into constant problems after major patches or updates. It was like clockwork and it always had to do with authentification. Steam and Zenimax know this is an ongoing issue. It's been an ongoing issue since the very first day ESO launched on Steam. Zenimax admits in their own forum that ESO generally does not to play well with Steam and it requires constant vigilance to keep after it.
After trying everything on my end as well as reaching out to Zenimax and my ISP, the only thing that fixed my issues was divorcing from Steam and using Zenimax launcher. I had no issues after that.
