Considering you're a conventional tank (not an offtank or hybrid or magicka tank ice staff etc.)
DK :
fiery grip -> unrelenting grip (the pull)
spiked armor -> hardened armor (default major resist buff)
dark talons -> chocking talons (default trash CC aoe)
Dragon blood -> green dragon blood (self heal, if you need it)
reflective scale (situational, there is one or 2 bosses in the game that might require it)
inhale -> deep breath (situational, if you need to interrupt a specific boss)
magma armor -> magma shell (default ultimate, the big panic button)
molten weapons -> igneous weapons (situational, if you really don't need much defence and have a free slot)
obsidian shield -> igneous shield (your main group defence)
ash cloud -> cinder storm (situational, alternative CC)
pick all class passives.
Sword and shield :
shield wall -> spell wall (situational, might be useful depending on boss)
puncture -> pierce armor (the main taunt)
low slash -> heroic slash (default ultimate generation buff, you need that tosustain your ult)
defencive posture -> absorb magic (this needs to be on your main block bar, it's your main counterspell)
pick all s&s passives
Heavy armor :
pick all passives. The active skill is situational (alternative to hardened armor if you really need the CC immunity for a boss).
Undaunted :
inner fire -> inner rage (the ranged taunt, since there are only 3 taunts in the game and one is on the ice staff, you need the other two, the melee taunt and this one, it's non negociable).
Bone shield -> Bone surge (very situational if you really need more group shields or specialize in a damage shield spamming build).
pick all passives
Assault alliance war :
War horn -> aggressive war horn (the main group buff, this one is mendatory 90% of the time,especially on DK, you have to do pvp to get it, and you need it as a tank).
Mages guild (optional) :
Equilibrium -> balance (alternative tospiked armor, better magicka sustain with it, use it if you want to be more support/buffer).
pick the passives if you use it
Psijic order (optional) :
By just having a psijic skill in a bar, you get a damage shield when you block if you have the passives. This is useful depending on how you like to tank.
undo -> temporal guard (situational, alternative ultimate).
meditate -> deep thoughs (situational, there is an interesting synergy with this skill, you get 30% reduced damage as long as you're meditatingif you pick the corresponding passive).
pick all passives if you use the line.
Vampire :
Vampire passives are very good for regen, if you want to be a vampire it's a very valid choice for tanking.
You don't really need to pick the active skills (you can, the drain gives you ultimate and the mist offer a good defence against boss attacks if you can't flee fast enough, but there are already a lot of good tank skills, might be more useful for magicka tanks).
About the stuff, the chest depends on the piece you want, all three chests can be of interest. About monster sets, chudan is good (it replaces the major resist buff, so it frees a slot for another skillif need be), bloodspawn for ultimate regen if you need.

I also have a nord dk she's vampire but I don't use any vampire ability nor psijic kinda prefer class abilities for front bar I run

Ultimate : war horn / magma armour
For passives all class , heavy armour , racial and one hand shield plus the undaunted ones as well as fighter guild and for gear I basically use plague doctor + ebon armoury + blood spawn and all my points into stamina with jewellery being mag recovery enchant and for food orzorga's red Frothgar with all this my stats are like
40k+ health
20k+ stam
2k+ mag recovery
34k spell and 32 physical resistance
And I never run out of resources!!
