If you have it disabled, the game won't allow you to target npcs.
On a related note, does disabling that stop people from forcing you into PvP? There have been a few times where high level players approached me in the newb zones and tried challenging me to a duel. When I rejected the duel they would repeatedly try and get between me and whatever mobs I was fighting PvE, like they were trying to get me to accidentally hit them (I have "allow attacking innocents" turned off).
Do I have to specifically turn on PvP or would they be able to trick me into PvP if that was turned back on?
Prevent attackinng innocents is only useful to avoid accidentally attacking a guard or other NPC while fighting nearby enemies and such, usually not a problem.
