Yes, the HUD was modified, although awhile ago now. One thing that can help you inhabit your character a bit more without checking your stats with TAB so often is to listen to the audio cues. A lot of information about your condition is communicated that way as well.
In terms of realism, the character notes when hunger, cold, or thirst are starting to set in. Even if you don't hear it, as I said, the icons appear before it actually starts to affect you.
If hitting tab (or Y-Left Bumper) is all it takes to break immersion for you, then I'm sorry for your condition and I'll pray for a speedy recovery.
Guess I'll wait for a HUD mod.
That being said, maybe it wouldn't be too hard to give players a HUD choice if they want the more intrusive, older version? I am not a programmer so I really don't know how much trouble it would be to give people the option.
Overall I prefer a minimalist HUD.
For me HUD's is that break the immersion.
The guy/girl are always saying "I'm hungry, thirst, tired, Is cold, this pack is heavy".
He/she complain all day. :))
But if you play in PC with controller or has one plugged, can use Joy2Key to make TAB be a ON/OFF command.
I use to press W - suffer tenosynovitis in left hand.
Macrogamer allow set macros on keys, not sure if has a kind of "toggle".

The "crosshair" doesn't show up, neither do prompts such as "open door" etc. Setting to always on also does not help.
It is an unspoken rule in the north to keep your complaining down to a dull roar as it robs you and the people around you of morale. This was a poor decision to place crucial information in that form. This is why it is so wildly unpopular. No one has anything against the great actors voicing these lines. It is simply a matter of psychological survival.
