Also, perhaps the ragdoll stats totals don't register things as quickly as you would expect. The weights on the clothing don't work right either. I am wearing 9+ kilos of clothing and registering only 7. It 'weighs' less on me than in my inventory ....

also...no storyline...Survival mode.
I'd suggest it doesn't matter which slot you can put the item in....rather, what that slot represents for your player character...in this case the inner or outer layers of clothing.
The Headgear and Accessory slots have been reversed for as longs as the new clothing UI has been out. The only 2 places where "closest to the body" actually means "outer layer" and "furthest from the body" means "inner layer". And it only becomes obvious if you equip one of the items that can only go in a specific slot, or you layer 2 of the same items, with drastically different conditions (1 new, 1 ragged toque for instance), and switch them around to see how the overall stats change down in the lower left section. Back in the earlier versions of the game, the Accessory slots were on the left side of the screen, the headgear was on the right side of the screen, when they switched them, the slots got reversed.
