Where do you go once supplies at Milton town run out? (Survival)
I am new so sorry if this a dumb question lol Like the game but it feels like there are no
structures or hardly any supplies in any other area than Milton town? like what do you do
end game or progression wise, seem's like after that town, no other area has much in it, unless i have missed them. Any help would be appreciated.

Most areas are "bare" for a reason. The game is more about survival against the elements and wild. I would start with story mode, where they push you to objectives and locations. This way you can learn the maps, learn howto survive.

Most of the Vets that still play this are pretty HC and are going after some real challenging achievements. Some I did not have full patience in getting, Achievements drive he game for a lot of people. For me, it is just about surviving.

There's plenty of structures, but Mountain Town (Milton) has the highest concentration.

What do you do? You keep going, you keep putting one foot in front of the other and try to survive.

Move from Mountain Town to another map. There are many, many maps outside of mountain town with many more supply areas and there are hidden prepper caches on several of the maps. Look at the world map in your inventory and it will show you which direction to go.

Take a good look at your crafting menu. In lots of ways it is a good guide to the goals you should be aiming for in order to survive in the long term. Running short on ammo? Clearly you want to acquire bows and arrows. Need arrows? You should leave Milton and find a forge in another region to make arrow heads. Etc., etc.

You play survival or Wintermute (story) mode?

If the second you are supposed focus and progress the story...

Otherwise arguebly any map allow surviving for very, very, VERY long time (however story mdoe is limiting in availabel crafting and tools for it)

A stroll through Mystery Lake, followed by a trudge across Pleasant Valley, leading to a jump up Timberwolf Mountain then swing by Ash Canyon ... that should keep you busy for a few days :-)

The whole idea is to keep moving and searching/exploring. Once you're familiar with the various regions and the locations in them, you'll be able to plan your journey better. You'll set up outposts and go to the relatively (or really) rare locations that let you do something special (like make ammo or create tools).

The thing with Milton/Mountain Town is it has access to three other regions, two of which are a bit harsh. Mystery Lake is good for loot but to get there you need to do some climbing. There are also some sustainable activities like fishing but it depends on what you're looking to do.

I recommend, if you don't have one already, you look for a bedroll. Climbing takes a ton out of you and I usually try to rest before so I've got full stamina before attempting a climb, even a short one. If you have coffee you can get the boost from that to make it but you'll pay for it later.

If you're okay with spoilers, there are guides with maps of regions so you can figure out how to get out of that region to find other ones. You might find you'll like another place better than that town though I do like the old lady's house. Very cozy. :)

This is good advice.

Mountian Town is the easiest area in the game and when I realized how easy it was I have always avoided going there unless its an emergency just because the painfully easy life you can live there makes the game boring.

I think the game is a lot more fun moving from area to area like a nomad, taking what you need and leaving what you dont immediately need, setting up outposts as you go.

Plus if you like base building (more like base clean up), then you get to scratch that itch multiple times. Each region usually has at least a couple of places that are ideal for one reason or another. Maybe one has a lot of storage or another has a full stove (gotta love roasting meat on six different spots). Usually there's a downside so it's never meant to be a permanent place to stay. That said, depending on your playstyle and interests, you could find a place like that.

If you're new, I definitely recommend heading to Mystery Lake. You have a way of getting there from Mountain Town but you do need to do some climbing (rope) so plan accordingly. You'll find a lot to loot and a map that's easier to explore (much of it can be walked to with only a few places that require climbing). There's also a nice surprise there and, if you're playing in any mode other than Pilgrim, a not so nice surprise.

One thing about transitioning: how you gear up will determine how well you survive. Remember that firewood can be replenished (though of course at a cost) and food will be consumed (freeing up weight) over time. You will find more gear but you also will encounter more of the same threats.

And like I said, outposts are helpful. If you find a great cave or place that you can hole up for in a transition area or along the way to another region, it's often worth leaving some stuff you can spare behind for emergencies (e.g. firewood, matches, water, a can). Food will rot so don't leave any though (not that it's really a spoiler but just in case: cooked meat left outside in the cold tends to last the longest).
