I will say that they are quite rare, more so than bears. Also, keep in mind that some locations are not guaranteed spawns. For example, one will only appear near Unnamed Pond if you see the moose rubbings on trees. I think the one near Trapper's is guaranteed although I have encountered it no more than three times in hundreds of hours of play time.
As for your final question, some people say that hanging around the spawn area all the time prevents them from spawning. I can't say for sure as I have not personally experienced. I tend to roam maps all the time, though, so I don't usually hang around any where.
ETA: I have found the one in CH to be the most reliable to spawn, even if it takes a while. Also, Ash Canyon moose spawns seem a bit more regular, but that might be a false impression since I have spent much less time there than on other maps.
And also, i've encountered moose spawned on a bridge after lighthouse in desolation point region with no trees near it. That was really scary.
That one in Milton basin once gave a real fight. took me maybe 6-7 arrows to kill him and few jumps from rocks :D
In CH I often see him near the garage. And rarest one, in my opinion, is in DP. saw him just once and that one broke my ribs.
The FM/Milton Basin vicinity is one, although there's also a wolf who likes to hang out on the side by Milton Basin.
The TWM Hut is another, although again there can also be wolves around.
Moose seems to migrate across regions. No way for me to know for sure but I think that the game spawns a moose randomly in a spawn location based on the whole world map. Excepting Bleak Inlet where the moose is the sole big game animal on the west side so it spawns (and migrates in a limited number of areas of that region), the game has 10 or 11 regions to choose from so it can be a long (or short) wait for it to reappear.
Moose don't migrate. They have set areas where they spawn, approx two per map, and they stay to these areas (with the torn bark)
TWM - in between the entrance and the Hut, and then in between the Hut and the Wing
AC - bottom of the triple-rope climb leading up to Mining Camp, and the Bitter Marsh forest
PV - Pensive Pond area, and the abandoned cabin area with the birch forest (near the Barn)
ML - right outside of Trapper's, and right outside the Dam
DP - only one: the bridge (with the waterfall near Matt's Truck)
etc.
