Im sure you can use 2 stims in a row if your health bar is low, it might be that if your health bar is full all the way you are not able to. (Or stamina bar) try with diff options, but i think only health bar can effect.
I have used a stim before when my energy was on zero.
Try drinking a cup of coffee after one stim looses its effect, that way you dont need to sleep right after using it.
Dont forget if you are encumbered (cant sprint) before using a stim, you can sprint after using a stim eather-only in case if you are to tired and cant sprint.

You really do not need to be carrying a ton of stuff on this challenge. always evaluate along the way what you really need. You shouldn't need to be carrying more than 30kg.
The stim gives you a straight adrenaline shot for short-term energy (Couple of irl minutes where your stamina bar will not go down no matter what you do) and a health boost. Then it wears off, and you get your stamina set to red (total exhaustion) but you keep the health boost. Your health bar doesn't matter (I have used stims just to reset my sleep cycle before), nor does anything else.
There are many tactical uses for the stim -
When dangerously low on health, the extra health boost and stamina boost can get you out of danger and keep you alive for long enough to save your run. Also when dangerously low on health the stim can give you a big jump-start on health recovery, so you can recover in a single night instead of it taking 3 nights or whatever.
The stim is really good for rope climbs, especially long ones like in the basin, or to the twm summit. This works best when you have coffee as well, so you can use the stim, do the climb or run or whatever it is you needed all of that energy for, and then use the coffee once it wears off to get all of your energy back.
In a REALLY desperate emergency, the stim can be used just for the simple act of getting from point a to point b as quickly as possible (you can sprint the entire time). Say you are caught out in a blizzard or surrounded by a pack of wolves or whatever, you can pop a stim and have the energy to run across half of any map in the game.
And as stated, you can use one to reset your sleep schedule. Any time you are wide awake when you really need to be dead exhausted, or just want your schedule adjusted back to where you are sleeping when you want to be, pop a stim, wait a bit, boom. Instantly tired.

At this moment I also think it could be only the health bar.
Because it is for sure not the fatigue bar. Yesterday I was on second rope in TWM could not use the stim so I took 5 coffees to get enough power and before climbing the rope I checked if I can now use the stim with fatigue bar half full, and I still could not use the stim. Mean time my heart/health bar was fully loaded.

I was thinking the same before I thought this was the key, but it is definitely not, since it did not worked two times, even if I drunk 4 or 5 coffees. I think I had this problem the first time because weather and cloth situation was so good, that I was never cold and health was fully loaded.
I am quite good at my speed run and I did it way over 50 times, and I just try to make even better times. Normally I carry around 13kg only even with 2,5 liter water.

This is my favorite challenge and after over 50 runs, I am really fast on it.
In the beginning I just worked with coffees, I found out how to finish it without sleeping one hour.
My target was to be faster than 24h. Then I learned the advantages of the stim, first only for last climb, then also for the double climb in PV. After that I used all remaining stims to run quick over the maps like the sea in CH, so I finished my next target to be under 20h.
Now I think stims are more important than coffee.
My best times are with only 2 coffee boxes (10 coffees) and 4 stims before summit.
So I started to use (and search) stims as much as I can since I can be so much quicker.
And then this problem came up, that I was in the middle of TWM and could not use another stim.
Now I need to understand the reason why in some situations I cannot use two stims in a raw and most times it is no problem.
Right now I think stim or at least second stim cannot be taken if health bar is fully loaded, but I am not 100% sure.
I really know the whole way so good, no matter if foggy, by night or if snowing, even in a blizzard most times I find my way. I think I also know each short cut possible, for example there is also a huge short cut between second last and last rope in TWM.
My best time so far is 18h 31m, and I think it is possible under 18h.
