The Last of Us was not a realistic game, no one can carry a Sledge Hammer, 2 M4A1, a M1911, enough ammunition, and food/drinks, without running out of stamina, or getting injured after carrying these equipment.
you can set hotkeys in 3.0, just click the item in your hands section of the inventory, then press a number, I think people will just get pissed since they died from something that they couldn't prevent, since as you said, this can't be detected.
NPC's are suppose to be other players, if you want NPC's go play in roleplay server, NPC's are more of a singleplayer improvement than a Multiplayer one, therefor making it less of a priority.
You do know you could have made your own thread in the suggestions sub topic, so why didn't you?
