That being said, I have a MacBook Air, and the game still runs perfectly fine. I can assign hotkeys for 3-9 and 0, and have no issues using all hotkeys from 0-9. Are you sure you're hot-keying the items correctly?
You should right-click the item in your inventory (one that can be held and is not a weapon), and then click the number you wish to assign it to. You may have to hold the Cmd key as you do so, but you probably won't need to.
As Molton suggested, get on Sierra, that's your best bet to start if you don't have it already.
That being said, it has been a while since I've messed with them, and I haven't touched them for gaming at all. One suggestion I've seen floating around is to make sure the items are in the 'hands' of your inventory. i.e. not in clothing, but that little section at the top.

I don’t use the Mac for gaming anymore, but it’s annoying to right click something and click on “equip” everytime. You know the feeling. I tried unturned on 2 same MacBooks air and both had the same problem. Hotkeys weren’t working. I guess no one cares what MacBook users do🤷♂️.
Im sorry, but good luck, I couldn’t find a solution for it.
