It allows you to select Hard mode (in addition to the other modes), and if you complete the game again with the other character, it unlocks One Dangerous Enemy mode.
I'd recommend keeping a separate save file for each character when loading up replays so that you can choose between having ODE on or off.
No, start an entirely new game without loading your previous Chris save. If you load from your previous save and complete the game, it will enable ODE on the next playthrough.
If you want ODE, finish the game one more time with Jill on the previous save (or vice versa) and it'll be enabled for future playthroughs on that file.
This may seem like a lot of playthroughs but there's always Real Survival and Invisible Enemy Mode to spice things up.
