achievement for completing the game only with a knife
I completed the game without using grenades and knives for Chris and did not burn enemies, but they still did not give me an achievement, and I also did not take a gun.

Try logging out of steam...then clear your cache...restart...log back in see if it triggers

Mind also the foot crushing of a crawling zombie's head, I read that disables the achievement too.

Had a playthrough yesterday to get it but got ambushed by one random Crimson Head while accidentally equipping a defensive item, no save.

I know you said you didn't use them, but you're not allowed to use defensive weapons either. By default, this use is automated and activated. So you should set it to 'manual' to be on the safe side.
As long as this option is enabled, this can happen to you - just as easily by accident as stepping on a zombie's head.

I didn't use it because I didn't even take protective items, but I think it's because of the difficulty level? I used the easy level, and as I understand it, achievements can only be obtained at normal and higher levels.

no easy too you can do no save achivemenet and knife only achivement work, Don't burn zombies and crush their heads with your feet
