Possible To 100% Game In Single Playthrough?
In the last 2 games you could acquire all runes, all weapon mods, all suit upgrades, fully unlock EVERYTHING in a single playthrough. Is that possible here... or should I save my gold?

It seems possible amho, depending of your skill and level dfiiculty set in the Options of the game...
well, maybe...assuming you value dumb achievements over fun. however, I just ran into an issue in chapter 6 where I found everything but it says 99%. I was playing on ultra violence and I don't know if that has anything to do with it. you'll probably be turning the game into easy mode to do it all in one sitting, but I'm not really sure if that even had anything to do with it or not. I honestly don't care just figured I'd give you a heads up that this happened.
When I played the previous 2, I would play on Ultra Violence or nightmare, and I actually did find getting everything pretty fun. Upgrading every weapon mod 100%, find all codex, all toys, pretty much 100% all collectibles, all secrets, all mission challenges, all weapon challenges... I found it quite fun. Dunno what you mean by valuing achievements over fun.

Same thing happened to me so I went back and looked i didnt finish one of the side quests the 3 quests you get for gold

Yes it is possible on a single playthrough

Edit, unless your going for all the skins and milestones as well, than it requires 3 three playthroughs at minimum.

I almost got 100% in my first playthrough.

Every level is designed so that you can get everything in that level with what you have the first time you do it. None of the secrets require a specific weapon and all of the challenges are for something you either get in that level or got in a previous level.
I finished the game in 25 hours 100% everything. There was only a single lever which i had to redo cause i missed something. But the second time you play a level you can speedrun most of the parts.

If you miss something in a level and need to replay it, do you have to complete the level to get credit? Or can you find the secret you missed and then exit out?

yes but you most keep your eyes on everything for easy to miss stuff

You don't need to complete the level. Just get to a checkpoint and exit to the main menu. Your progress will save.
i didn't 100% my first run but i wanna say technically yes if you keep reloading checkpoints for mission/mastery challenges, same as the others

Most likely a challenge you didnt finish. Had the same happening to me :)
Very hard, you miss one item and it's no longer 100%.

You can leave as soon as you collect the thing. Did that for about 5 levels.
