How does Co-Op work?
Can you just play online with 1 other friend (so, a total of 2)?

Can play with one other person, with up to three other people, both in the campaign and horde mode. Can play with 2 people fine, will be the two of you and no bots or anything.

Horde Mode scales accordingly, in campaign you can personally set it. When you start a game, you have several options, choose what stage to play, choose what character to play as, but the host of the lobby can also choose the difficulty of the game (three different difficulties), and the number of zombies. The number you can choose to scale to be set for 1 to 4 players, or a special Elite option for more zombies than scaled to 4 people. So for 2-people, you can scale the zombies to 2 people, unless you want an additional challenge, then you can scale it to more.

No, but you can scale the zombies for a single person, and there's certain things that only work in single player (the X-ray kill cam, where you see a bullet go inside a zombie in slow-mo and see an x-ray view as the bullet punctures bones, arteries, etc., is only available in single player, can scale the frequency of this in the options menu. In the third episode also, there's some mini-games you can partake in that are only available in single player).
