Old question but you have to go into Setup and and go to advanced mouse settings.
There you turn off "Analog Mouse moving" that did it for me, I was going crazy!

Yes, as M@tEo$ said.
These settings are in the Setup.exe not the actual in-game settings that are somewhat limiting compared to the main game Setup program.
To stop the player moving back or forth while moving the mouse the setting "Analog Mouse Moving" needs to be un-ticked or Off.
I was literally going crazy by this and the settings should be off by default, imo.
Hope it helps the both of you and future players.
EDIT:
In your Steam folder go to: steamapp \ common \ One Unit Whole Blood.
Inside that folder there should be a filed called SETUP, as M@tEo$ said, drag that file onto the DOSBOX.exe file the one with the orange and yellow icon.
This way you should be able to access the SETUP.exe file through the Dosbox emulator and also from there you should go to: Controller Setup\Setup Advanced Control Options\Advanced Mouse Setup and make sure, I believe, Mouse Y Axis is empty.
It should say "Analog_Moving" and this entry can be deleted by pressing the Del button on your keyboard. Mouse X Axis should say "Analog_Turning" and I believe that should be left alone and staying as it is.
If you don't see any of this or even see the folder inside the Steam folders you might have the Fresh Supply version of this game and then I believe the setting is off by default or even removed all together.
Update:
I went and tried Blood using Luxtorpeda again right after this post and it worked perfect. Although I'm still getting the ScummVM cartoon like screen with Thief and Tomb Raider III, still getting nowhere there. Thanks.
The DOS Blood has an issue with the mouse free look being you can't move vertically AND horizontally at once
There's a little tool called BMOUSE for this, if you wanna stick to DOS Blood
