You may have been lucky - or unlucky - before. I have this happen all the time when tending on the field, since that's usually how I skill up all my non-primary medics. It's good to give all your soldiers at least a little medical skill on the side if they're capable of it, not just for missions but you never know when your primary caretakers may come down with the plague or malaria or an ax in the back.
They'll be out tending those hapless raiders when someone who was at like 81% pain and had a pinky shot off has that stump bandaged and suddenly they feel like they're okay to just limp off the field leaving a thick trail of blood as if they're not surrounded by armed enemies and the person tending them decides to riflebutt the jerk until they stay down. If they're lucky, none of the others have enough time to aim their weapons and fire.
