Any idea what that is? I am told it must be a room with at least 25 tiles with no work / rec facilities and the like. No other details. I made a fully floored room of that size but I still get the message that Form of Life is missing. What am I missing?

Make the 'Form of Life' at the art bench and then put it in the room.

This. I'd check your ideology template when you have a minute. You likely have some altars, statues, etc that are either required or can be added to aid rituals
