Sterile tile usage
I was wondering, for sterile tiles to 1) help in surgery rooms 2) speed up research
does the whole room need to be tiled in sterile (expensive, 12 silver each) tiles? or just under the table/bed and around it?

Or just not having a dedicated cleaner and all of your pawns have more important tasks atm. Which ofc just underlines your point but since I personally hate wasting a pawn for cleaning (and sometimes my slaves doing that job "quit") I can see that situation arise ^^

Not really. A few butchered animals is all it takes to get blood everywhere, and with the fact that you're using a dirt floor with its -1 cleanliness along with the butcher's table, it doesn't take much to get it to -2.

Besides which, other than the minor cost associated with it, and the flammability (which is basically irrelevant in the kitchen since the stove is flammable), straw matting is just better than dirt floor anyway.

From what I can find, food poisoning chance starts increasing below -1 (with -1 itself being safe), not -2. So a pure dirt floor is acceptable, but nothing more.

You know that there is a devmode, right? You don't have to spread unverified rumors, you can check it yourself.

2% for -3.4 cleanliness, 0% for -2 and 11x11 is still far in safe territory even after you butcher a herd of muffalos. Add flooring, make kitchen-freezer large enough to store meaningful amount of food, don't butcher creatures so hard that you can't even store their products in your kitchen-freezer and you'll be fine.

Nope, I didn't know it was there, but I do now. You didn't state where you got your info from and it's the first I've heard the -2 number (anything under 0 seems counterintuitive to me tbh), but it looks like that is the case. The more you know.
