It's amazing how often I hear that kinda thing 'round here. For years now.
Well, with 10 seconds of thought it might be pretty obvious as to why.
It makes the game more complex for new players. Some of which are already dissuaded by the game's complexity. As for veteran players, the mods already take care of this, and veteran players usually know how to work around it even without a mod, so this makes the priority of adding it in "vanilla" low.
First, yes you adjust your home area to only cover the things you need cleaned. Repairs won't work on items outside of that (as you say), but you can manage that by keeping things close or adjusting your home area as needed - kinda like you probably already do with fires.
Also, adjusting cleaning as a separate area from the home area is really only necessary after a considerable period in the game. That's also one more area for a new player to manage and understand. Using the home area makes this intuitive for new players.
Would it be super difficult to implement? Nope. That doesn't mean it fits the design goals of the base game.
Also, as mentioned, mods have this covered already making such an implementation a pretty low priority. I could see it happening one day though. In the mean time, I just use cleaning priority. Works fine.

But adding a zone for cleaning is too complex and may confuse the player ... mmmkay
If you're worried about repairs, limit your home zones in affected areas strictly to the objects themselves. For external walls, have the zones on the walls and remove the designation elsewhere.
If it's about cleaning unnecessary areas, create an inverse zone and exclude the areas you don't need your dedicated janitors/cleansweepers to clean. That keeps the areas available for repairmen to attend to. I usually designate only the interior of my superstructure base for my cleansweepers so everything else is excluded, and may exclude barns if necessary.
If it's about firefighting, put firefoam poppers where you've excluded the area from the home zone, such as barns. Put them everywhere important anyway since it's a good thing to have.
If it's about cleanliness, keep in mind that you only really need certain areas totally filth free, like the lab, hospital, and kitchen. The rest won't descend into anarchy just because nobody has cleaned it in the past ten minutes.
With all of these I've never used a dedicated cleaning zone feature.
