Bed orientation?

What's the ideal way to pack beds in? I've heard there has to be side-accessable to the pillow for treatment, but I've also seen people pack them in side by side both directions too. Just looking for the most efficent way.

Well the most efficent bedroom would be a 2x1 with a single bed or 2x2 for a double bed... but this is a bit... extreme..

Place the pillow towards the door as this is how they get in/out of bed, and also where the doc goes to treat.
