Nope, the hospital is spacious, beautiful, sterile, and my colonist is also completely healthy. I couldn't make a more perfect scenario without the editor if I tried.
i have no problems.
maybe check your doctor pawn specifically under "i" and look for his/her surgery success chance. not every lvl 20 doctor is the same.
-Clean room. Use sterile silver tiles and make sure its literally being cleaned if it gets dirty. No animals should enter this room.
-Use hospital beds with attached vitals monitors.
-Make sure its at least 50% lit. Having a standard lamp near the bed is fine.
-Make sure your doctor has decent skill. 10+ is ideal. Also check their manipulation and consciousness levels. If they can't see well or use their arms well, they will perform poorly. This can be offset with drugs like wake-up, for instance. Bionic eyes and arms also help.
-Use at least the mid-line medicine and not herbal medicine.
-Consider saving before the surgery and reloading if you fail if you really don't like the gamble of it.

I'll do some testing to figure out what's going on, I don't think the mods I have would ♥♥♥♥ with the doctoring but not much else would make sense either.
Still fails about 1/4 of all surgeries.
No mods at all. Something did break, but savescumming is the workaround.
