Surgery Fail Chance seems busted.
I started a new colony as soon as I could purchase the new DLC. After playing said colony up to this point, I didn't have a single successful surgery until this morning. I have lost 23 kidneys, 14 lungs, and three colonists before only now having a single pawn with a wooden hand. I get that the colonists aren't infallible but is it not ridiculous to have multiple catastrophic failures in a row with a success rate above 100%

Well your surgeries could be failing due to lack of light, cleanliness or your doctors manipulation skill. Sight also plays a huge role in manipulation.

Nope, the hospital is spacious, beautiful, sterile, and my colonist is also completely healthy. I couldn't make a more perfect scenario without the editor if I tried.

seems not normal tbh.
i have no problems.

maybe check your doctor pawn specifically under "i" and look for his/her surgery success chance. not every lvl 20 doctor is the same.

For best results:
-Clean room. Use sterile silver tiles and make sure its literally being cleaned if it gets dirty. No animals should enter this room.
-Use hospital beds with attached vitals monitors.
-Make sure its at least 50% lit. Having a standard lamp near the bed is fine.
-Make sure your doctor has decent skill. 10+ is ideal. Also check their manipulation and consciousness levels. If they can't see well or use their arms well, they will perform poorly. This can be offset with drugs like wake-up, for instance. Bionic eyes and arms also help.
-Use at least the mid-line medicine and not herbal medicine.
-Consider saving before the surgery and reloading if you fail if you really don't like the gamble of it.

If you claim the success rate is above 100% and you understand the multipliers involved, then it sounds like you have a mod breaking something. The cap on success chance when doing a surgery is 98%, a fail 1 in 50 times is expected with a good doctor using good meds, and it's safe to assume that's what everyone else is seeing since nobody else has made a thread like this.

I'll do some testing to figure out what's going on, I don't think the mods I have would ♥♥♥♥ with the doctoring but not much else would make sense either.

I noticed with organ harvesting this last 24 hours of play...fully lit operating room with master patient bed and vitals monitor, sterile floors, skill 15 surgeon with 115% manipulation and 100% eyesight.

Still fails about 1/4 of all surgeries.

No mods at all. Something did break, but savescumming is the workaround.
