I would guess you have colonists from multiple ideologies, one of them is multiple spouses, one of them is not, the people believing in the polygamy ideology keeping hooking up with people who don't who then feel cheated when they move onto the next one. Only unmodded situation I can think of where a lot of "cheating" might happen, and I'm not even sure it works that way.
The wife is a baroness with chemical fascination, giving away slaves to the Empire for honor and binging on hard drugs, both of which the husband hates.
The husband is an industrious by-the-books man who works during the day and sleeps during the night. More often than not the wife is too high on drugs to sleep, so they don't even share a bed.
Both are heavily modded to appear beautiful, both have 15+ social, and both get a lot of fans trying to hook up with them. Still, there was no cheating. The colony is 8+ years old.
But there are a lot of breaks-up due to proposal rejections. Don't know what's up with the others, but nobody wants to marry in this colony.
Can someone explain how that comes about? Don't they usually just break up and get together with someone else? That's no affair though.

It's an incredibly low chance to happen when a pawn flirts with a pawn who is very attracted to them and already in a relationship but that relationship is in a very bad state. I don't know the numbers offhand, but if you recruit a colonist who is married to a neutral/allied faction and never sees them and the relationship stays locked at the baseline 30 for married, cheating basically doesn't even happen in that state and that colonist will remain faithful and "alone" probably for the entire playthrough even with all colonists implanted with aesthetic parts and maxed out relations with each other.
When it does happen you get a yellow alert message titled "Affair!" along with a warning sound. The original couple does not directly break up or divorce over this, there is just a large -70 opinion offset for the pawn that was cheated on. If you stack body modder with aesthetic parts then you can keep couples together at 100 relations even with an ongoing affair. Very rare but it was possible to basically have polygamy before ideology.
Mods? I know there are several trait mods that add a trait that makes pawns cheat. RJW also handles cheating very differently from vanilla and it depends on your settings for that mod.
