Cooked food has a chance based on the cooking skill of the cook and the cleanliness of the room, which is much lower with a clean room and good cook (note that the butcher's table is inherently dirty, you should have it in a separate room from your stove)
Meals made by the Nutrient Paste Dispenser NEVER cause food poisoning.
Also, it didn't cause "Food poisoning (major)", it caused food poisoning. Which will go through 3 stages: initial, major, and recovering, with different levels of penalty at each stage.
If you're going to eat corpses, butcher them first and make them into meals. It'll make your not-cannibals less unhappy, you get more food out of it, and you get leather.
You probably want to pay more attention to mood though. Food poisoning is the least of your issues after eating a raw human corpse.
Also, I don't have any mods or DLCs. I got the game less than 24 hours ago.
Eating raw food - there is always small chance, even with berries.
Your cook sucks - and incompetent cooking made the dish poisonous.
Dish was cooked in very dirty environment.
How to avoid? Cook food obviously - play with job priorities so only your best cook is allowed to use the stove. Built a stove in a separate room, with flooring, away from butchering station, because blood splatters count for calculating the dirtiness.
Have somebody on higher priority when it comes to cleaning, so filfth does not have time to accumulate.
I play this game for the same reason I play CK3. Not to win, but to do stuff which would get me imprisoned in real life.
