Except for "Prison Labor" none of them interacts with warden behavior. But even if one of my mods screws with the game, there are still people complaining of the same thing who is running vanilla.
Just create a new food restriction, "clear all", then select exact what you want them to eat. I usually select raw insect meat, but you can treat your prisoners a bit better.
There are two possible solutions to this problem:
1. Remove some mods.
2. Get another mod to fix the mods (pinned answer).
Just tried it in vanilla, still the same issue. So proberbly a bug rather than a mod conflict.
And again, still don't have any mods that interact with warden behavior.
I've never had a problem with this, and I don't use any mods. Perhaps what PolecatEZ said about prisoner medical beds is important - I may not have noticed that happening. The warden might be following the rules, but the doctor isn't.
I retract what I have said.
