They picked the lock on the door using materials they got from meals delivered.
There was a fork, spoon, or spork.
Perhaps the lock is not a traditional lock and they managed to use a hoop of their own hair that was braided and kept hidden during inspections.
Maybe when the Social character was indoctrinating the inmate they managed to get a pen or pencil off of them.
In any case there are ways to reduce the chances and the effectiveness of escapes.
Example, Make the prison cell better aka put in beauty items.
Put in drugs or alcohol so that the prisoner can get smashed or stoned on their own btw not coerced.
Add implants such as Joywire so that long term stored prisoners are really really happy and have reduced consciousness.
Check on the information panel to see how soon a prison break actually is and when its getting close have the doctor knock them unconscious.
All of the above, you should not do btw.
My headcanon interpretation is that they stole a key or had time to fashion a lockpick - and once they're out in corridors etc, those doors don't have deadbolts or anything preventing them from being opened from inside the building, and so the prisoner manages to make a break for it unless my people intercept them.
