ther is an option where you can harvest organs without any mood debuff. but only pawns with that ideology are effected from that. all others will still suffer the mood debuff for harvesting organs.
2.) Did the colonist doing the surgery have the "Pyschopath" trait? If so, they won't get a debuff if they harvest an organ.

4.) On Rimworldwiki com, it mentions that consciousness and movement will be lowered by poor breathing.
5.) IIRC when you schedule a surgery, the doctor will apply the medicine for the surgery and then they will get the hediff for being affected by anesthetic, lowering their consciousness until it wears off. You don't need to do anything else- 10% Conscious is normal and they will die if you try to lower it to 0.
Wooosh so I got lucky on my own colonist then? She had 1% consciousness after smokeleaf when I operated on er. I thought less consciousness would be better during surgery...?
Why is there an option to administer drugs then?
The option to administer drugs is primarily for incapacitated pawns who suffer from poor mood or even withdrawal symptoms, and you want them to be able to get drugs to counteract that.
